We’ll schedule an appointment for your initial consultation, listen to your specific needs, and pair you with the right therapist on our team.
Please only fill out one appointment request. Filling out duplicate forms may cause delays. Due to the high demand for mental health services, please allow up to 48–72 hours for our Team to process your appointment request.
If filling out this request for a minor, please enter the minors name and date of birth with your email and phone number so that we will be able to pre-verify the insurance in order to avoid delays.
As an outpatient mental health counseling practice, Palmetto Counseling LLC is unable to provide emergency psychiatric treatment or crisis stabilization services for patients who are in immediate crisis and/or danger to self/others.
Please be advised that all emergency and crisis issues involving the immediate safety and well-being of a patient and life-or-death concerns require you to immediately contact 911 and/or report to the nearest Emergency Room without delay.

Palmetto Counseling & Consulting Services participates with many major commercial insurance plans; however, we do not participate with Medicaid. See our complete list of accepted providers and employee assistance programs.
Insurance participation does not guarantee coverage for every service. Benefits vary by individual plan and may include deductibles, co-payments, co-insurance, exclusions, limitations, and authorization requirements.
Family, couples, and marital counseling may be subject to different coverage requirements than individual psychotherapy. These services may involve relational or family circumstances that are appropriately documented using ICD-10-CM Z-codes. Depending on your insurance plan, services associated with relational/family Z-codes may not be reimbursable when the plan requires a covered mental-health diagnosis for behavioral-health benefits. When insurance does not cover the requested service, self-pay will be required.
We strongly recommend contacting your insurance carrier before your first appointment to confirm that your specific plan is accepted and to verify coverage for the specific type of service you are seeking, including family, couples, or marital counseling (and all types of relational counseling).

If a service is not covered by insurance, Palmetto will discuss applicable self-pay fees before services are provided whenever possible.
