For your convenience, we have provided the forms below for you to sign and/or review before visiting Palmetto Counseling. All clients are required to complete these forms.

To make your first appointment easier, we have three options available to you:

  1. Electronically complete your registration paperwork using our Client Portal. You may need to ask our Front Desk staff for your login credentials. The client portal can be found here.
  2. Download and complete the Client Intake Form and the Client Signature Consent Form and bring them with you to your appointment. The download links can be found below.
  3. Arrive 30 minutes early for your appointment to allow time to complete all the registration paperwork in our office.
